History

Founded in 2005, Southern Crescent Home Care, LLC or SCHC is a healthcare provider of the activities of daily living to individuals who are not capable of functioning on their own. The Georgia Department of Health/Health Care Regulation Division licenses SCHC as a Private Home Care Provider to provide skilled nursing, personal care, and companionship. We have also been a Community Care Services Program, or CCSP provider member since 2006.

We strictly adhere to the rules and regulations of the Georgia Department of Community Health and other Medicaid Waiver Programs. it is the policy of SCHC to hire a qualified staff to stay on this course. Client’s satisfaction is key to the success of SCHC.
